The thyroid is a small gland in the neck that controls how fast the body uses energy. Thyroid problems are common in Pakistan, especially in women, and in areas where the diet has been low in iodine. A simple blood test can find most of them.

Compare prices for the TSH test, free T4 or the full thyroid profile.

The three main tests

  • TSH (thyroid stimulating hormone): made by the pituitary gland in the brain. It is the best first test. When the thyroid is underactive, TSH goes up; when it is overactive, TSH goes down.
  • Free T4 (FT4): the main thyroid hormone in the blood.
  • T3 / free T3: the active hormone; mostly useful when an overactive thyroid is suspected.

A thyroid profile usually includes TSH, T4 and T3 together.

Typical normal ranges

Each lab uses its own range, printed on the report. Typical adult values are:

TestTypical adult range
TSHabout 0.4 to 4.0 mIU/L
Free T4about 0.8 to 1.8 ng/dL (10 to 23 pmol/L)

Ranges are different in pregnancy, where tighter targets are used, and in young children.

Underactive thyroid (hypothyroidism)

High TSH with low free T4. Common signs:

  • Tiredness, feeling cold, weight gain.
  • Dry skin, hair fall, constipation.
  • Heavy or irregular periods, difficulty getting pregnant.
  • Low mood and slow thinking.

It is usually treated with daily levothyroxine tablets.

Overactive thyroid (hyperthyroidism)

Low TSH with high T4 and/or T3. Common signs:

  • Weight loss despite eating well, feeling hot and sweaty.
  • Fast or irregular heartbeat, shaky hands.
  • Anxiety, poor sleep, loose stools.
  • A swelling in the neck (goitre) or bulging eyes.

Treatment may include medicines such as carbimazole, chosen and monitored by a doctor.

Tips for accurate results

  • Fasting is usually not needed, but follow your lab's advice.
  • If you take levothyroxine, many doctors prefer the test before that day's dose.
  • Biotin (in many hair and nail supplements) can upset thyroid results. Tell the lab, and ask your doctor whether to pause it for a couple of days before the test.

Price and labs
